Registration & Required Forms
The OSAA’s 2020-2021 calendar provides for three distinct seasons from late February to late June with limited overlap between seasons. Click here to view the updated OSAA activities calendar.
Each season will feature an equally shortened regular season with adjusted contest limitations, followed by an OSAA Culminating Week. Specific plans for culminating week events will be developed in conjunction with member schools in the coming months in alignment with large group gathering guidelines issued by the state. In order for Crescent Valley High School to be eligible to participate in boys and girls basketball, wrestling, and cheer for the 2020-21 school year we need to meet all of the conditions set forth by ODE. This includes, any county designated as High Risk or Extreme Risk needs to have submitted the Outdoor Contact Sports Opt-In Form for approval. Prior to participation, all athletes need to have the following on file in the CV Athletic Office:
- Code of Conduct (reviewed and acknowledged by athlete and parent/guardian each year)
- Athletic Registration Form (NEW: online! Form required each season)
- Physical Exam Form (Expires 2 years from date of exam. Please mail, scan/email, or drop off form in CVHS office)
- Participation Fee $113 (fee waived free & reduced lunch) per sports season (2020-21)**DO NOT SUBMIT PAYMENT UNTIL NOTIFIED BY THE CV ATHLETICS DEPT.**

To participate in an athletic program, students should contact the head coach or athletic director before the beginning of the season for information regarding tryouts, practice schedules, and team requirements. Fees pay for our coaches, transportation, referees, etc. If you have any questions or are in need of financial assistance, please call the athletic office at 541-757-5809.
A physical exam is required every 2 years and for first-year athletes and new students. Student athletes must also satisfy the academic and training rules of the OSAA and Corvallis School District.
Fall sports kick-off the new school year for athletes, the school community, and spectators. Some fall sports, like boys and girls soccer, have tryouts in the summer. There may also be summer conditioning and practices. To find out specific practice and game times as well as other important information, contact the team coach.

The winter athletics season typically gets started in early November with tryouts and practices. To find out specific practice and game times and important information, contact the coaches. Athletics staff will check your eligibility (grades & credits), collect your user fee, and make a clearance slip for the new sport.

Spring athletics are often impacted by rain and wet fields. There are times when practice or a game may be cancelled due to weather conditions. Non-School Sponsored Sports
Community sponsored club sports are defined as those sports not recognized by the Oregon Schools Activities Association. They are independent of and receive no funding, services, or supervision from the Corvallis School District. The use of a school name by a community sponsored club sport will be at the discretion of the principal after review of the completed application. The district retains the right to revoke the use of the school name at any time. Links provided are not hosted by CVHS or the district and the district assumes no liability for the activities of community sponsored club sports.
